Create My Support Plan: Top Tips


Before you start, here are some tips for getting the best from the Support Planner:

1 Take your time

Thinking about what you want to do in life and the support you need can take time. 

You can save your plan at any time, then finish it later. So have a tea break, or come back tomorrow… it’s up to you!  

2 You do not need to do every section

Before you start, take some time to find what is in each section and what they are about. You only have to complete the sections YOU think are important.

If a page or section does not apply to you, then you do not have to fill it in.

3 Add as much or as little detail as you want to.

This is YOUR plan, its about your life. Add what is important to you, but you only need the detail you feel you need to and are happy to share.

A Support Plan should be about what you need and want from life, in your own words.

4 Ask for advice

If you are using Create My Support Plan on your own, it is helpful to talk to other people about what you put in your plan. This could be family, friends, support workers, professionals or anyone you know and trust.
