Create my Support Plan is flexible and can be used at any time in your life. Developed in 2013 by the West of England Centre for Inclusive Living (WECIL), a charity of disabled people in Bristol and funded by the Department of Health, it can be used by individuals and professionals alike.
It is free for everyone to use and for every email address; you can have access to one plan.
Create my Support Plan allows you to create a support plan you can give to your social worker during the care assessment processes; find services rated by other people with our national directory and some useful factsheets on finding the right support for you.
